Educational & Professional Background
Dr. Everold Haffizulla graduated from the University of the West Indies College and Medical School prior to completing his Family Practice Residency at the University of Miami/Jackson Memorial Hospital.
Dr. Haffizulla is five times board certified in Family Medicine and is a fellow of the American Academy of Family Physicians. He is a volunteer Preceptor for University of Miami School of Medicine, Nova Southeastern allopathic Medical School and their Physician Assistant Masters program as well.

Dr. Jason is a graduate Emory University’s Bachelor’s Degree program prior to graduating from the university of Miami’s medical school. He is two times board certified in internal medicine, having completed his residency at the Cleveland clinic.
Dr. Jason Haffizulla is a volunteer Preceptor for University of Miami School of Medicine, Nova Southeastern allopathic Medical School and their Physician Assistant Masters program as well.

Dr. Nigam Parikh
Educational & Professional Background
Dr. Nigam Parikh completed his Medical School training at M.P. Medical College of India. He is board certified in internal medicine, having completed his residency at St. Vincent’s hospital in Massachusetts.
Dr. Parikh is a volunteer Preceptor for University of Miami School of Medicine and Dr. Kiran C Patel College of Allopathic Medical School, Nova Southeastern University.
Rheumatology
Dr. Kristin Chai
Educational & Professional Background
Dr. Chai is a rheumatologist. She specializes in musculoskeletal conditions and autoimmune diseases, including rheumatoid arthritis, lupus, systemic sclerosis and osteoporosis as well as fibromyalgia.
Dr. Chai received her Bachelors degree from Boston College prior to graduating from the university of Miami’s medical school. She also completed her internal medicine residency and rheumatology fellowship at Jackson memorial hospital in Miami.

Katrina is a certified Physician Assistant. She is a graduate of Nova Southeastern university in Davie, where she completed her master’s degree in medical studies, majoring in Physician Assistant studies.
Fred Appelkvist
Educational & Professional Background
Fred Appelkvist is an Ultra sonographer at our office. He has been with Medical Associates of Tamarac since 1990. He specializes in procedures such as Echocardiography, Vascular and General Sonography, and Nerve Conduction Studies.
